MEDICAL CAP CLOSURE SPECIALIST POSITION

We are seeking a dedicated and experienced Gap Closure Specialist to join our team and play a vital role in ensuring the delivery of top-notch healthcare and maintaining regulatory compliance.

 

The Gap Closure Specialist will be responsible for identifying and addressing gaps in patient care and working on attestation initiatives to improve patient outcomes and maintain compliance with healthcare regulations.

 

This role requires a deep understanding of healthcare processes, strong analytical skills, and the ability to work collaboratively with cross-functional teams.

 

Key Responsibilities:

 

MUST HAVE WELL MED CHAMPION EXPERIENCE

 

Gap Identification and Assessment:

  • Review patient records and clinical data to identify gaps in care and potential areas for improvement.
  • Analyze patient charts, lab results, and medical histories to determine gaps in care.

 

Gap Closure:

  • Develop and implement strategies to close identified gaps in patient care.
  • Collaborate with healthcare providers to ensure timely and effective closure of gaps.

 

Attestation Management:

  • Assist in the collection and organization of data required for attestations and quality reporting.
  • Ensure that all attestations are accurate, complete, and submitted in a timely manner.

 

Quality Improvement:

  • Participate in quality improvement initiatives to enhance patient care and meet regulatory requirements.
  • Monitor and track performance metrics related to gap closure and attestation.

 

Documentation and Reporting:

  • Maintain detailed records of gap closure efforts and attestation activities.
  • Prepare regular reports on gap closure progress for internal and external stakeholders.

 

  • Compliance and Training:
    Stay updated on healthcare regulations and requirements related to gap closure and attestations.
  • Provide training and guidance to staff members on gap closure and attestation procedures.

 

Qualifications:

  • Prior experience working in a Tier 1 medical office or healthcare setting
  • Strong knowledge of medical terminology, clinical processes, and healthcare regulations
  • Proficient in data analysis and record-keeping
  • Excellent communication and interpersonal skills
  • Detail-oriented and organized with the ability to meet deadlines
  • Preferred Qualifications: Certification in healthcare quality or related field
  • Experience with healthcare information systems and electronic health records (EHR)

 

Job Type: Full-time

Salary: $15.00 – $18.00 per hour

Expected hours: 40 per week
